EAP Health Clearance instructions are country specific. Please read the following chart carefully to complete all the necessary requirements for your EAP program.
*Please note that for London/Paris or Rome/Madrid multi-site programs you can select either of the two countries, the instructions should be the same.
*Students doing the Argentina/Chile multi-site should select Argentina for country and list Argentina on the Travel Clinic.
*Students doing the Mediterranean Food multi-site (Spain/Italy) should select Spain or Italy the for country, the instructions should be the same.
*Students doing the UC Sacramento/Mexico City multi-site should select Mexico for country and list Mexico on the Travel Clinic.
Please be sure to refer to your pre-departure checklist to see when your health clearance is due to the UCEAP Systemwide office. Please note this date is often different than the date to finish your clearance through Student Health.
** A malaria prophylactic agreement is required for Botswana, Ghana, Senegal, and Tanzania.
Student Health now has 24/7 phone service to enable students to speak with Registered Nurses about any concerns or questions needing medical attention. Students can access the nurses though an option on our regular SHS phones, or by calling directly to 800-539-1387. The service is open to any student who calls, regardless of whether they are insured by Aetna through GHI or not.
There are three after-hours phone lines that students can use to access professional help 24 hours daily: SHS’s Nurseline at 800-539-1387, CAPS 24/7 Counseling at 805-893-4411, and CARE Confidential Advocacy for Sexual Violence at 805-893-4613.
