Private Physician Appointments

This option is recommended if: you have never been seen at Student Health Services before and you have a private provider. If you do not complete SHS online option in time, you must see a private physician.

The private option is not recommended if: You only go to Student Health with your health concerns. If this applies to you, see SHS Online Clearance instructions.

The student may chose the option that is most convenient for them. *Students going to the following countries cannot see a private physician and must do the SHS online option: Botswana, Ghana, India, Senegal, South Africa, Solomon Islands and Tanzania.

For students who do not have a doctor in the area, and you have missed the Student Health Online Clearance Deadline. Student Health recommends you visit one of these clinics:

Please note: You will need to ask for a Statement of Health Clearance and your UCEAP paperwork to your appointment. Unless noted on your health clearance instructions you do not need a physical or vaccines and will be charged extra for these services.

**The health clearance is different than a routine physical and will not be covered by insurance. You will likely have to pay out of pocket**
 


WARNING: Students who do not follow these instructions, wait until the last minute, lose their forms without enough time to replace them, or otherwise miss the deadline will be dismissed from the program.

Print out the EAP Confidential Health History Form and the UCEAP Health Clearance Form

Fill out the EAP Confidential Health History Form before your appointment

30 minutes +time to gather immunization records

Make and attend and appointment with private physician AND any specialist* you are seeing for an ongoing condition. Bring both of the above forms with you. This clearance may NOT be done by a family member.

Mail the original of the UCEAP Health Clearance Form to UCEAP office 60 days before program start date stictly inforced

*If you are currently under the care of a specialist (other than the physician you are seeing for the clearance) for medical or psychiatric condition, you will need to request a signed letter on official letterhead approving your travel and detailing any special accomodations or care you may need abroad. Please submit the letter from your specialist with your Health Clearance form. Specialist Letter Request Form (PDF)

If you have any physical impairment, learning disability or other condition, for which you will be seeking accommodation abroad, have your campus Disabled Students Office send a memo to the UCSB campus EAP Office and the Universitywide Office of the Education Abroad Program indicating the condition and your needs. Universities abroad will require this memo before special arrangements — i.e. granting extra time for tests, arranging for special facilities or assistance — can be considered.
